Medicaid enrollment assistance is available for adults and children who are blind or disabled and anyone over the age of 65 and their families in need of the following Medicaid programs offered through New York state:
- Community Medicaid (including those with excess income and/or resources)
- Medicaid for home care services
- Medicaid for nursing home care or other services in an in-patient setting
- Medicaid for individuals in need of waiver services (OPWDD, Traumatic Brain Injury, Bridges to Health and other HCBS programs, etc.)
- Medicaid for working people with disabilities who want to buy into the program
- Medicare Savings Program for those with Medicare that need help paying Part A, Part B, and/or prescription drugs
- Medicare enrollment assistance for those also eligible for Medicaid
Take three steps to find out if you, or someone in your family, is eligible for one of these Medicaid Programs.
Step 1: Find a site near you
Review our calendar of upcoming enrollment sites throughout Long Island where you can meet with an Application Assistance Enroller near you. We also have appointments available on a limited basis at our Hauppauge office. Call 631.435.3000 to check availability.
Step 2: Meet with an Application Assistance Enroller
Application assistance is held regularly at various locations throughout Nassau and Suffolk counties. All of our site locations operate on a first come, first serve basis. If you are unable to attend, you can send an authorized representative.
Step 3: Collect the necessary documents
Our Enrollers will advise you about documents required to show proof of age, identity, home address, and income. Depending on the coverage you are seeking, you may be required to provide proof of your resources.
