Self-Screening for Medicaid Enhanced Services
Medicaid Members can receive assistance connecting to social needs services
If you are enrolled in Medicaid and you or your family are looking for support with food, transportation, or housing, there may be help available to you. For those who may benefit from additional supports, you may work with someone to get connected to the services that are right for you, depending on health status and need.

The Health Equity Alliance of Long Island (HEALI), is a Social Care Network (SCN) connecting community-based organizations and partners to support equitable health and life outcomes for our neighbors.
To get help and confirm eligibility for services, contact a HEALI Navigator for assistance at HealiHelp@hwcli.com or 516-505-4434. Navigators will also ask you some questions to help identify where you might need some extra support.
